DAY HABITATION SERVICES

DESCRIPTION

skills Orange County AHRC provides Day Habilitation services at several sites throughout the county. The participants of Day Habilitation spend much of their time out in the community at various locations and practice and learn many skills. Whether in the community setting or at a site, the focus is on skill development and social experiences. Relationships are built and new opportunities and experiences are explored. Our participants range in age from young adults, age 21, to senior citizens. The scope of individual needs is broad from those requiring minimal supports and assistance to those who require a significant amount of hands on care for their daily activities. Our staff are highly trained and have access to specialized training and adaptive equipment to provide the compassionate care our participants require. We work closely with the participant, the family and care providers and the Medicaid Service Coordinator to develop a Day Habilitation plan which will address the desired outcomes of the individual.

ELIGIBILITY CRITERIA

Enrollment in the Home and Community Based Service Medicaid Waiver (HCBS); Documentation of the developmental disability prior to age 21; NY Medicaid; approval from the Hudson Valley DDSO.

Adult guyOrange County AHRC has been providing adult day services longer than any other agency in the county. We believe each individual is unique and deserving of opportunities to enhance and maintain skills through a combination of site based and community experiences. We provide an array of clinical services including nursing, physical therapy, occupational therapy, speech therapy and psychological services at our primary locations. We also provide transportation services to and from a site using our own fleet of vehicles and certified, qualified drivers. At our program sites, individuals learn skills which include those for self care, grocery shopping and food preparation, interpersonal relationship, acceptable social behaviors, good health habits, gardening and many cognitive skills. They participate in the community through a variety of activities including volunteer deliver of Meals on Wheels, Friendly Visitor shopping, delivering daffodils for the American Red Cross, bringing musical entertainment to schools and nursing homes, and collecting donations for the Hudson Valley Food Bank or for local animal shelters. They also go to local stores, shops, museums, art centers, restaurants, banks, libraries, movies, plays and a whole host of other community facilities where they not only practice skills, but have lots of recreation and fun! Our trained staff work along side the participants to provide the guidance and encouragement needed for learning and the praise appreciated for success! For those persons needing intensive hands on care, our staff assure that the daily needs of each person are met with dignity and respect.
