The Driver is responsible for the safe and timely transportation of consumers in an agency vehicle to and from program and special events.

The Driver is responsible for the safe and timely transportation of consumers in an agency vehicle to and from program and special events.
Supervision and Scope:
Transports approximately 10-14 consumers per run.
Principle Responsibilities (Illustrative Only):
- Inspects vehicles daily and notes needed repairs as required including tires, lights, brakes, etc.
- Appropriately maintains all required documentation including mileage logs, attendance records, and D.O.T. check lists.
- Ensures the safety of the consumers being transported; responsible for the safe operation, security, and cleanliness of the assigned vehicle.
- Performs routine checks on assigned vehicle such as oil level, tire pressure, etc.
- Assists consumers in entering and exiting vehicles as necessary.
- Responsible for appropriately handling all transportation relate matters in relation to established transportation operating procedures.
- Assists consumers moving around agency premises and on trips.
- Provides crisis intervention when necessary.
- Other duties as assigned.
Working Relationships:
Maintains working relationships with program staff, consumers, and parents
Supervisor:
Directly supervised by the Vehicle coordinator.
Qualifications:
A valid class 2 or 4 NYS drivers license, 19 A, and CDL certifications.
AND
- The ability to understand and retain and appropriately implement written and verbal instructions.
- The ability to physically lift or assist consumers in moving around as well as the ability to physically intervene with consumers when necessary.
DIRECT SUPPORT WORKER I, II, AND III
Job Summary, Purpose and Distinguishing Features:
Works directly with consumers in an independent manner performing whatever actions/tasks are necessary for the proper implementation of the functional program. Operates independently for most of the work shift with access to Level Supervisors, Residence Manger, or Clinical Staff for guidance as necessary.
Job Title: DIRECT SUPPORT WORKER I, II, and III
Job Summary, Purpose and Distinguishing Features:
Works directly with consumers in an independent manner performing whatever actions/tasks are necessary for the proper implementation of the functional program. Operates independently for most of the work shift with access to Level Supervisors, Residence Manger, or Clinical Staff for guidance as necessary.
Supervision and Scope:
Provides support and training to assigned consumers. May provide guidance to Director Support Assistants, substitutes, volunteers, and or summer youth workers.
Principle Responsiblities (Illustrative only):
- Works directly with the consumers in the appropriate completion of actions and tasks, which fulfill the requirements of the functional program.
- Acts as an appropriate role model for consumers in working towards the overall goals of independence, integration, individuality, productivity, and consumer choice.
- Ensures that program rules and established schedules are adhered to.
- Responsible for the appropriate documentation of consumer programs and the expenditure of monies.
- Administers medication in an appropriate manner and completes supporting documentation after successful completion of medication administration course.
- May accompany consumers on outings and field trips to provide community awareness training.
- Supervises and trains consumers performing such tasks as cleaning, grooming, light maintenance chores, leisure activities, etc.
- Responsible for knowing the operating policies and procedures of the assigned program and handling all matters accordingly.
- Participates in staff, team and in-service meetings.
- May assist consumers in moving around and may physically intervene with consumers when necessary.
- May transport consumers on trips into the community in agency vehicles and provide medications as necessary.
- Other duties as assigned.
Working Relationships:
Maintains working relationships with clinical and medical staff, transportation staff, and Level Supervisor and Residential Services staff.
Supervisor:
Directly supervised by the level Supervisor or Resident Manager.
Qualifications:
High School Diploma or Equivalent.
AND
- Ability to understand and retain verbal and written information.
- Ability to effectively implement appropriate action based upon written and verbal instructions.
- Ability to communicate effectively (oral and written) with consumers, staff, families and the public.
- The ability to physically lift or assist consumers in moving around as well as the ability to physically intervene with consumers when necessary requiring current SKIP and CPR certification. (new 7/03)
- Successful completion of the Orientation period.
AND FOR WORK IN THE RESIDENTIAL PROGRAM:
- Valid N.Y.S. Driver's license.
- Successful completion of the Medical Administration Course and appropriate driving Certification. ****Upon completion of the Orientation Period and attainment of 19A, and Medication Administration certifications the Direct Support Worker I will be eligible for promotion to Direct Support Worker II. If CDL driving certification is required and attained the individual will be eligible for promotion to Direct Support Worker III.
SUPERVISOR

Qualifications:
High School Diploma or equivalent
AND
- Two years experience working with mentally retarded people.
- Demonstrated ability to verbally communicate clearly and effectively with consumers and staff.
- Ability to communicate clearly and effectively in writing
- Ability to understand, retain, and implement appropriate action based on verbal and written information.
- Flexibility, poise, tact, and the ability to develop a good working relationship with individuals of various backgrounds.
- Ability to work with others as a member of an interdisciplinary treatment team.
- Willingness and openness to learn new approaches to dealing with mentally retarded people.
- Ability to be flexible in ones work schedule and, if necessary, on short notice so as to be available at times when a consumer is in need of special intervention.
- The ability to physically lift or assist consumers in moving around as well as the ability to physically intervene with consumers when necessary. May be promoted to grade 16A if is required to have maintain and use Medication and Driving Certifications
LICENSED PRACTICAL NURSE

Job Title: LICENSED PRACTICAL NURSE
Job Summary, Purpose and Distinguishing Features:
The Licensed Practical Nurse, under the supervision of a Registered Nurse, provides, monitors and records health care and related functions for assigned consumers/students.
Supervision and Scope:
Participates as a member of the interdisciplinary team.
Principle Responsibilities (Illustrative Only):
- Under the supervision of a Registered Nurse, coordinates medical information for admission and provides comprehensive nursing assessments for the intake process.
- Under the supervision of a Registered Nurse, provides and documents first aid to consumers/students, and as necessary, assists in the transfer to an appropriate medical facility.
- Under the supervision of a Registered Nurse, administers, records, stores and as necessary, transfers medication in a manner which will fulfill medical, state, agency and safety regulations and guidelines; monitors effects of medication for potential adverse reactions.
- Under the supervision of a Registered Nurse, provides comprehensive annual assessments as well as nursing and medication assessments for assigned consumers/students.
- Under the supervision of a Registered Nurse, develops and implements appropriate nursing care for consumers/students in need and appropriately maintains related documentation.
- Under the supervision of a Registered Nurse, maintains and ensures the accuracy of health related records and documentation including: medication, seizures, special incidents, daily log, etc.
- Assists the Registered Nurse in relation to staff, team and in-service meetings to ensure consideration of and provide information about medical and health issues as well as fostering healthful living and facility conditions.
- Other duties as assigned.
Working Relationships:
Maintains working relationships with program staff, medical practitioners, parents/guardians of consumers/students.
Supervisor:
Professional supervision provided by the Registered Nurse, administratively supervised by a unit director.
Qualifications:
Currently licensed to practice as a Licensed Practical Nurse by the New York State Education Department.
AND
- Good oral, writing and communication skills.
- Flexibility, poise, tact and the ability to develop rapport with individuals of varying backgrounds.
- Currently valid New York State Drivers License.
MAINTENANCE WORKER

The Maintenance Worker is responsible for the timely and appropriate completion of assigned cleaning, repair and maintenance duties.

Maintains assigned property(ies) or portion thereof in an acceptable manner.
Principle Responsibilities (Illustrative Only):
- Performs routine cleaning duties such as: sweeping, vacuuming, washing, stripping and waxing floors, general cleaning, etc.
- May make routine repairs and improvements including: plumbing, electrical, heating, carpentry and masonry work.
- Maintains grounds including: as applicable, lawns, trees, shrubs, brush removal, parking areas, sidewalks, driveways, etc.
- Assists with snow removal and sanding.
- May be required to respond to emergency repair situations as they occur.
- Assist with the moving of supplies and equipment.
- May perform routine task related to preventive maintenance systems.
- Responsible for the security of tools and supplies used.
- May be on call and carry pager during weekends and evenings.
- Other duties as assigned.
Working Relationships:
Maintains working relationships with all staff at assigned property(ies).
Supervisor:
Directly supervised by the Maintenance Manager.
Qualifications:
Good working knowledge of appropriate processes regarding property cleaning, repair and maintenance.
